Dr. Howard does accept medicare assignment. Most common insurances also are accepted. He does not see Workers Compensation cases. You are responsible for paying co-payments at the time of your office visit, and we will request this of you. If we do not participate with your insurance, payment in full is expected at the time of your visit unless other arrangements have been made. For your convenience we do accept Visa and Mastercard payments.

 

Dr. Howard does not do disability evaluations or fill out functional capacity forms. Other forms that need to be filled out are done by our staff and are billed for. Our staff will be happy to discuss the charges with you.

 

If your insurance company requires pre-authorization for office visits or treatments, you must obtain this prior to your scheduled visit. It is your or your referring doctor's responsibility to make sure this is done. You cannot be seen in our office until this pre-authorization is accomplished.

 

Please be sure to sign in at the front desk when you arrive at the office. If you cannot make your appointment please give at least 24 hours, preferably 48 hours notice so that other patients who wish to be seen can be worked in.
